The Possibilities Forum, a disability sector conversation by Mosaic Community Care and Westcare IncorporatedMosaic Community Care, in partnership with Westcare Incorporated, invites disability sector professionals to attend The Possibilities Forum.

This is an in‑person event for people working across disability services who want practical information, seek shared learning, and form stronger connections across the sector.

The forum brings together practitioners, leaders, and experts to spotlight complex and evolving sector changes and challenges, with a focus on real‑world experience and everyday practice.

What is the forum about?

The Possibilities Forum is a guided conversation about current challenges and changes in the disability sector.

Through structured discussions, the forum will explore how people working in disability services can better support participants, families and organisations in a complex and ever-evolving system.

The first forum will focus on two key areas that often raise questions and challenges.

Fireside chat 1: Navigating complex transitions – Hospital to Home

This topic will explore:

  • How to support timely and safe hospital discharges using NDIS supports
  • Interim funding options
  • How to identify assistive technology needs
  • Strategies for complex psychosocial presentations.

Meet the speakers

Fireside chat 2: Workplace adjustments to support inclusive employment

This topic will explore:

  • What workplace adjustments are and why they matter
  • Supports and funding available for people with disability and employers
  • How employment services and providers can work together
  • Ways to improve long‑term disability employment outcomes

Caroline Yates, Director, SN Services

Caroline Yates is a Director at Specialist Nursing Services (SN Services), a Perth-based provider specialising in complex and high-acuity care across multi-funding environments. With a strong clinical and operational background, Caroline has led the development of innovative service models that support individuals with significant medical needs to transition safely from hospital to home. Her work focuses on bridging gaps between the health and community systems, building capable workforces, and delivering person-centred care in highly complex settings, including Supported Independent Living (SIL).

Caroline is passionate about improving outcomes for the people we support, families, and providers by strengthening governance, clinical excellence, and practical service delivery. She brings real-world insight and leadership experience to conversations about navigating risk, complexity, and continuity of care during critical transition points. Caroline is also the Founder and Chair of the WA Complex Care Alliance (WACCA), where she advocates for improved systems, collaboration, and outcomes for individuals with complex care needs across Western Australia.

Kim Adamson, Chief Operations Officer, Mosaic Community Care

Kim Adamson has over 15 years’ experience in the community services and disability sector. She has led large teams and managed quality, risk, and accreditation programs across disability, aged care and out-of-home care.

Kim holds an MBA from the Australian Institute of Management WA (AIMWA), and has built and improved systems for major organisations and supported staff through operational change and review processes.

Her work includes guiding teams through audits and accreditation for standards, including the Out of Home Care Standards, National Principles of Child Safe Organisations, National Disability Standards, National Standards for Mental Health Services, and NDIS Practice Standards and Aged Care Standards. Kim is passionate about contributing to organisations who serve our communities, ensuring customer-focused service delivery, and driving financial sustainability through innovative solutions.

Georgia Miller, National Manager, JobAccess

Georgia Miller has over 15 years’ experience in occupational rehabilitation and disability employment services. As a qualified occupational therapist, Georgia has built a strong clinical foundation that enables her to assist people with disability achieve their vocational goals. She has leveraged her clinical and management expertise to inform her work as the Advisory and Employment Assistance Fund Manager and now as the National Manager in the award-winning JobAccess program on behalf of the Department of Social Services.

JobAccess is the national hub of workplace and employment information for people with disability, employers, and service providers. JobAccess can help people with disability with financial support and access to services to support them in the workplace. This support includes a telephone advice line and assistance organising workplace adjustments and modifications through the Employment Assistance Fund (EAF).

JobAccess has an expert team of allied health professionals who provide holistic and tailored advice on workplace adjustments to employers and people with disability from the first point of contact. JobAccess also has an Employer Engagement service – the National Disability Recruitment Coordinator (NDRC) – who works alongside larger employers to increase their disability confidence through free and tailored partnership agreements.

Fitz Cass, COO – ADE, Westcare Incorporated

Fitz brings many years of experience across the Care industry and Commercial sector, with a strong focus on supporting people with disability. His career spans roles from frontline support to senior leadership, complemented by lived experience through a close family member with significant disabilities.

He has served as a board member and chair within the not-for-profit sector and contributed by serving on various business and industry boards. Fitz previously led a franchised Home Care business, with franchisee across Australia, worked in management of disability employment agencies including a mental health service and later operated his own small business, giving him firsthand insight into the challenges of business ownership.
