Referral form

We welcome referrals from Support Coordinators, disability support professionals, other agencies, guardians, family members, and people with disability. If you are looking for support with any of our services, please fill in our online referral form and one of our team members will be in touch to discuss the next steps.

Share your feedback

Good or bad, we always welcome constructive feedback. It is important that we know if you, or someone you know, have been treated badly or unfairly. All submissions will be investigated and treated in the strictest confidence.

If you’re a client, you can also speak to your Coordinator or any member of our team – this is usually the quickest and easiest way to resolve an issue. Alternatively, please complete our feedback form, providing as much information as you can.

Speak up safely and confidentially

Mosaic is committed to providing safe, ethical and high-quality services. If you have a serious concern about misconduct, safety, compliance, fraud, or practices that may place people at risk, you can submit a confidential report through our Whistleblower Form.

Reports can be made by participants, families, carers, employees, volunteers, contractors and community members. You may choose to remain anonymous, and all reports are reviewed in accordance with Mosaic’s Whistleblower Policy.

Your voice matters. By speaking up, you help protect the safety, rights and wellbeing of the people we support.
