Support Coordination (Level 2)

Specialist Support Coordination (Level 3)

Our Specialist Support Coordinators aim to reduce barriers so you can access the NDIS-funded supports and services you need. They are qualified and experienced practitioners who offer a higher level of support coordination.

This allows them to work with people who have more complex needs and who may find accessing NDIS support challenging. Our Level 3 Specialist Support Coordination team use local knowledge of a range of systems and services to help you navigate through a variety of situations.

Specialist Support Coordination can help in situations such as:

  • Experiencing homelessness
  • Transitioning out of the hospital and into your home
  • Complex health support needs, including mental health
  • Living in remote areas with limited support
  • Involvement in the justice system
  • Children in care or transitioning out of care

Psychosocial Recovery Coaching

Psychosocial disability refers to a disability that may arise from a mental health issue. The word ‘disability’ is used because the mental health challenges you are experiencing can have a disabling effect on your life. Having a psychosocial disability may make you eligible for an NDIS plan and funding for supports and services to meet your specific needs.

Our Psychosocial Recovery Coaching provides specialist support to help you find and access the mental health services that best suit you. Your coach will help you build your capacity to recover and increase your resilience. They can help you to:

  • Find out about different services and supports and how these can help you
  • Access support from mental health services
  • Better understand and be able to navigate the NDIS

How we work with you

Our priority for NDIS Support Coordination is to get to know you. By understanding your challenges, your goals, your trusted support circle, and any service providers you currently utilise, we work with you to develop a support plan within the first month. This plan will include information on your goals and support preferences.

Your Support Coordinator will check in with you regularly to see how your supports and funding levels are tracking. Should your current plan no longer meet your needs, we will work with you and your support circle, gather the right information and help you with the plan review process.

Getting started is simple.

  • Step 2: We learn about you. One of our team members will contact you to discuss your goals, support needs and current situation. 
  • Step 3: Meet your Coordinator. We’ll match you with a NDIS Support Coordinator who is suited to your needs and circumstances. 
  • Step 4: Build your support plan. Together, we’ll identify your goals, review your NDIS plan and develop a plan for moving forward.

Frequently asked questions

Do I have Support Coordination in my NDIS plan?

Support Coordination is a ‘Capacity Building’ support. To determine if you have it, look on your NDIS plan for a section that outlines your Capacity Building funds. If Support Coordination is listed there, then you have it in your plan. If you’re unsure, contact your NDIS planner or local area coordinator for assistance.

Do I need NDIS funding for Support Coordination?

For Support Coordination and Psychosocial Recovery Coaching services, you need to have National Disability Insurance Scheme (NDIS) funding or equivalent.

If using NDIS funding, you must have the following line item included in your NDIS plan:

  • Capacity Building – Support Coordination and Psychosocial Recovery Coaches

How quickly can I start Support Coordination with Mosaic?

Mosaic is currently accepting new referrals. Once we receive your enquiry, a member of our team will contact you to discuss your needs and next steps. 

Will I have the same Support Coordinator?

Yes. Wherever possible, you’ll work with the same Support Coordinator so you can build a strong and consistent working relationship.

Our Support Coordination team works closely together to ensure consistent service quality and continuous improvement. Rest assured, your services will continue uninterrupted if your Support Coordinator is on leave or unavailable.   

How often will my Support Coordinator contact me?

This depends on your needs and the level of support in your NDIS plan. Some participants need frequent support, while others may only require occasional check-ins. 

Can my family, guardian or nominee be involved?

Yes. With your permission, we can work closely with family members, guardians, nominees and other trusted people in your support network. 

I’m not happy with my plan and supports. Can a Support Coordinator help me?

Yes. Your Support Coordinator can assist you in reviewing your current plan and supports. They can help you identify areas for improvement, connect you with appropriate services and guide you in requesting a plan review to better meet your needs.

What is Support Coordination?

Support Coordination is a service that helps you understand and implement your NDIS plan. It involves connecting you with appropriate services, assisting with service agreements, and guiding you through the NDIS system to achieve your goals.

What’s the difference between Support Coordination and Plan Management?

A Support Coordinator helps NDIS participants to understand the NDIS and build capacity to exercise choice and control over the services and supports that are right for you. A Plan Manager is responsible for organising payments, processing invoices and tracking budgets.

What’s the difference between Support Coordination and Psychosocial Recovery Coaching? 

Both Support Coordination and Psychosocial Recovery Coaching can help you implement your plan and connect to services. However, Psychosocial Recovery Coaching is more tailored for people with psychosocial disabilities as it emphasises mental health recovery, personal goals and resilience-building.

How is a Specialist Support Coordinator different from a Support Coordinator?

A Specialist Support Coordinator is an experienced practitioner who offers a higher level of support coordination. This allows them to be able to work with people who have more complex needs. They are adept at helping to navigate through a variety of situations, systems and services including those in health, education or justice. Our Specialist Support Coordinators aim to reduce barriers so you can access the NDIS funded services and supports you need.

What do Support Coordinators do?

A support coordinator’s key responsibilities include helping you understand your NDIS plan, assisting in choosing and connecting with service providers, coordinating services, advocating for your needs, and providing guidance on managing your plan effectively.

How do I choose a Support Coordinator?

It’s essential to find someone you trust and who can explain things in a way you understand. When choosing one, you might take into consideration factors such as their experience, how they understand your needs, how they communicate with you and how comfortable you feel around them.

Can Support Coordinators help me manage a crisis? 

Support Coordination is not considered a crisis service. However, support coordinators can assist with crisis management by connecting you to appropriate services and support during emergencies or when you need immediate help.
