From beginnings as an organisation started by three families, we’re now proud to provide opportunities for independence across over 20 locations in Perth metro.
With a growing range of services, including Positive Behaviour Support, we look forward to continuing to create possibilities and transform lives.

We are committed to improving the lives of people between the ages of 18 and 65 with an intellectual disability and their families.
Our team of highly experienced, positive and passionate support staff work to create personal one-on-one relationships with our clients in a caring and nurturing environment.
Mosaic is a proud advocate of social and community inclusion; integrating our homes into local communities through groups, associations and individuals who wish to engage with and support our clients.

As a registered provider of the National Disability Insurance Scheme (NDIS), we work in the home, in our facilities and in the community to develop tailored disability support plans for our clients. Our personal lifeplans ensure that we always create the best possible outcomes for individuals living with disability and enable them to have control over their personal growth, development and quality of life.
We were first established in 1992, and originally known by the name GACLA (Gosnells & Armadale Community Living Association).
GACLA was formed by a group of dedicated community members who were interested in developing a range of improved living options for their family members living with disability.
In 2001, GACLA changed its name to Mosaic, as this was deemed to be more reflective of the many people who receive integral support from us, and how each of these clients form components of the bigger picture that is our organisation.
In less than a decade, we have been fortunate enough to expand our services across the Perth metropolitan area, with the opening of several new places of residence for our clients in the southern suburbs of Brookdale, Baldivis and Harrisdale.
Since the start of 2018, our supported accommodation in Perth has grown by almost 20%. This impressive acceleration of growth has enabled us to provide our clients with more opportunities to assist them in living the life they want. In 2018, we relocated our head office from O’Connor to our new address of 2 Sabre Crescent in the southern suburb of Jandakot. Our new premises is our main hub, organising the provision of all-encompassing support services for Western Australians living with disability.
