At Mosaic, we help people with disability aged 18-65 secure the home, housemates and 24/7 support they need to live, actively, independently and safely in the community.
We’ve been providing fully supported accommodation since 1992. Today, we have homes across Perth, from tidy one-bedroom homes to spacious seven-bedroom homes.
But this isn’t just about finding somewhere to live. This is about living your best life.

So, if one of our existing NDIS supported independent living homes doesn’t work for you, or if it does today but doesn’t down the track, we’ll work with you to create a solution that does. We have an extensive range of supported disability accommodation options across Perth and around Rockingham.

Managed by experienced Coordinators, our teams use our proven person-centred approach and our Positive Behaviour Support framework.
This ensures we always carefully meet your unique needs and preferences, while openly communicating and consulting with you and your family on an ongoing basis.

Our team will work with you to build your skills to live as independently as possible. This includes assisting with:
- supervision, safety and security
- daily routines that work for you
- personal care (showering, dressing, etc)
- meal preparation, cooking and cleaning
- behaviour and social skills support
- medications and medical appointments
- getting out and about in the community

If you have Specialist Disability Accommodation (SDA) funding, our team can help you get the most out of it.
We partner with reputable SDA providers, so we can help you establish an SDA home and provide your day-to-day Supported Independent Living support too.
