How does it work?

A Specialist Support Coordinator offers a higher level of support coordination and is a qualified and experienced practitioner who assists clients with complex needs. The role involves support with managing the challenges in the person’s support environment which may include, health, education, or justice services. The role is involved in reducing barriers to accessing NDIS funded services and supports.

A Specialist Support Coordinator has a strong understanding of the NDIS and local knowledge of mainstream and disability services and supports available to clients, and the ability to navigate through complex situations and systems.

  1. Your Mosaic NDIS Support Coordinator will get to know you and understand your current supports – the important people in your life and your current service providers.
  2. You and your Support Coordinator sit down, plan for the future and work out a service agreement.
  3. After three months and again after six months, we’ll get together with you to review your plan and how things are going.
  4. As the end of your current plan approaches, you and your Support Coordinator will reflect on what’s working and what can be improved. We’ll put a report together that includes information from all your service providers.
  5. When we review your plan and put a new one together, we’ll discuss whether you still need help with NDIS support coordination. It may be that you feel confident enough to manage and coordinate your own NDIS plan, support providers and service agreements.

Different types of NDIS support coordination.

There are three types or levels of NDIS support coordination:

Support Connection (Level 1)

This is support that helps you by building your ability to connect with informal, community and funded supports. It’s all about helping you get the most out of your NDIS plan and pursue your goals.

Support Coordination (Level 2)

We offer this level of Support Coordination. Your Support Coordinator helps you build the skills you need to understand, implement and use your NDIS plan. It’s about making sure you have a range of support services that help you maintain relationships, manage service delivery tasks and live more independently and actively in your community.

Specialist Support Coordination (Level 3)

We also offer Specialist Support Coordination. This level of support coordination is ideal if you have more complex requirements and need specialist support services.
