Mosaic Community Care is committed to providing safe, ethical and high-quality services. We encourage people to report serious concerns about conduct, governance, safety, compliance, fraud, misconduct, or practices that may place participants, staff, volunteers, or the organisation at risk.

This Whistleblower Form provides a confidential way to raise concerns that you believe should be investigated. Reports can be made by employees, participants, families, carers, volunteers, contractors, or members of the community.

You do not need to have proof of wrongdoing before making a report. However, we ask that you provide as much information as possible to help us understand and investigate the matter. You may choose to remain anonymous, although providing your contact details may assist us if further information is required.

All whistleblower reports are taken seriously and will be reviewed in accordance with Mosaic’s Whistleblower Policy. We are committed to protecting people who raise concerns in good faith from victimisation, retaliation, or unfair treatment.  Use this form to report serious concerns about misconduct or wrongdoing at Mosaic.
