We provide you with personalised support. A key part of our approach is the Community Coordinator, who will:

  • Get to know you and your goals
  • Work with you to plan meaningful community experiences
  • Connect you with local opportunities and services
  • Continuously refine supports based on your feedback

They lead your planning and engagement, ensuring your supports stay relevant and valuable over time.

Once your plan is in place, our Support Workers will help bring it to life. They will:

  • Support you to access the community
  • Help build confidence and independence
  • Encourage participation in activities you enjoy

Your Community Coordinator stays involved throughout, making sure everything continues to work well for you.

How our Community Access & Support service works

  • Step 1 – We get to know you: We take the time to learn about your interests, your goals, your support needs, and what is important to you.
  • Step 2 – Create your plan: Your Community Coordinator works with you to develop a personalised plan that reflects your goals and aspirations.
  • Step 3 – Find the right Support Worker: We carefully match you with Support Workers based on your goals, interests, and preferences.
  • Step 4 – Get involved in your community: Whether it’s sport, volunteering, study, work, social activities or learning new skills, we’ll support you to participate in ways that are meaningful to you.
  • Step 5 – Review and adapt: As your goals change, your Community Coordinator will continue working with you to make sure your supports remain relevant and valuable.

What types of Community Access supports do you provide?

Mosaic provides personalised, one-to-one support to help you access the community and achieve your goals.

Where you want to go and what you want to do, is up to you – maybe take up a sport or hobby, volunteer, work, study, hike, go to the gym, or simply get out and about to shops, cafes and appointments.

What are the eligibility requirements for Mosaic’s Community Access & Support?
Mosaic supports people across Perth’s southern metropolitan region to build confidence, increase independence and participate in their community.
Every person’s circumstance, goals and support needs are different, so we assess enquiries on an individual basis to determine whether Mosaic is the right fit and what supports may be available.

Do I need NDIS funding for Community Access & Support?

Community Access & Support can be funded in different ways, including through the NDIS and other funding arrangements.

If you’re unsure what funding you have available, or how supports may be funded, our team would be happy to discuss your options and help point you in the right direction.

Can Mosaic help if I’m changing providers?

Yes.

If you’re considering changing providers, our team can help you understand your options and guide you through the process.

We’ll take the time to understand what’s important to you, your goals, and the supports you’re looking for.

If Mosaic is the right fit, we’ll work with you to make the transition as smooth as possible.

What happens after I contact Mosaic?

Once you get in touch, one of our team members will take the time to understand your goals, support needs and circumstances.

We’ll answer any questions you have, discuss what you’re hoping to achieve, and explore whether Mosaic is the right fit for you. There’s no obligation to proceed.

Can I choose who supports me?

We aim to find people who are a good fit and who can support you to achieve what matters most to you. We take the time to understand your interests, goals, preferences, and support needs before matching you with Support Workers.

I’m not sure whether Mosaic is the right provider for me. Should I still get in touch?

Absolutely.

We’re always happy to have a conversation and help you explore your options.

Even if Mosaic isn’t ultimately the right fit, we’ll do our best to point you in the right direction and connect you with services that may be able to help.

How quickly can supports commence?

Timeframes can vary depending on your goals, support needs, and location. Once we understand what you’re looking for, we’ll discuss realistic commencement timeframes and next steps with you.

Where in Perth does Mosaic offer support?

What if I’m not sure what I want to do in the community?

That’s okay. Many people come to us knowing they want to be more involved in their community but aren’t sure where to start. Our team can help explore your interests, goals, and strengths to identify opportunities that are meaningful to you.

Can you help me find some community activities that suit me?

Yes. If you have some specific activities or services in mind that you are interested to do or access, your Support Worker can help you to find the right ones that suit you and help you achieve your goals. If you want to be more involved in the local community, but don’t really know what you want to do, we can help you identify something that will be a good fit for your interests and abilities.

Will I always have the same Support Workers?

Our approach is very personalised. That means we carefully match you with just the right Support Workers who will go on this journey with you – people who share your interests and get what you want to do and why. If we need to, we’ll even recruit someone who suits you!  Of course, there may be occasions when they are not available…for instance when they are on leave. In those cases, we check to see if you are OK with someone else supporting you in the short term.

Do you have any Support Workers that speak languages other than English?
