We work with you to develop your personal life plan, supporting you to build goals and then achieve them.

Our person-centred community access service supports clients to explore opportunities at home and in the community. Our suite of services includes in-home care as well as support to explore and develop skills in areas such as learning and education, recreation, social and employment.

Activities we support include:

  • Maintaining and developing friendships and community connections;
  • Participating in sport and recreational activities;
  • Accessing educational and training courses;
  • Life skills training such as: housekeeping, meal preparation, transport training, money management and shopping;
  • Assistance attending medical, dental, therapy and other appointments.

There really is no limit to what you can achieve.

It's as little or as much as is right for you.

  • Support can start straight from school. We work with you, your family and school to develop the plan best suited to your needs.
  • Services are available 24 hours a day, seven days a week. We cater for all ages from young adults up and are happy to start chatting before you leave school.
  • With a diverse range of highly skilled support team members, we work closely with you to match the right team members in order to meet your support requirements.
  • Our staff have a broad range of qualifications across aged care, childcare and education assistance and disability services, allowing Mosaic to provide high-quality support services.
