Our Community Access and Support team helps people with disability aged 18-65 set and achieve their personal goals.

Making new friends. Taking up a sport or hobby. Socialising or volunteering. Pursuing further education, getting a job and building a career. Or just getting out and about, doing shopping and keeping appointments.

Whatever you have in mind, our experienced Support Workers will develop an evolving life plan with you and provide the friendly support you need to realise your goals, safely and enjoyably.

We provide you with personalised support. Which is why we carefully match you with just the right Support Worker – someone who shares your interests and gets what you want to do and why.  If we need to, we’ll even recruit someone who suits you.

From when you meet with one of our Home Coordinators and get to know your Support Worker, your plan, your goals and your experiences are our focus.

We’re here to encourage you and support you every step of the way, nurturing the skills and confidence you need to do the things you want to do at a pace that suits you. It’s all about you!

Frequently asked questions

What types of community access supports do you provide?

Mosaic provides personalised 1:1 support to help you access the community and achieve your goals. Where you want to go and what you want to do, is up to you – maybe take up a sport or hobby, volunteer, work, study, hike, go to the gym, or simply get out and about to shops, cafes and appointments.

What are the eligibility requirements for Mosaic’s community access and support?

To be eligible for community access with Mosaic, you need to be aged 18+, be located south of the river, and have NDIS funding. To check if your funding covers community access, please get in touch with us on hello@mosaic.org.au.

Can you help me find some community activities that suit me?

Yes. If you have some specific activities or services in mind that you are interested to do or access, your Support Worker can help you to find the right ones that suit you and help you achieve your goals. If you want to be more involved in the local community, but don’t really know what you want to do, we can help you identify something that will be a good fit for your interests and abilities.

Will I always have the same Support Workers?

Our approach is very personalised. That means we carefully match you with just the right Support Workers who will go on this journey with you – people who share your interests and get what you want to do and why. If we need to, we’ll even recruit someone who suits you!  Of course, there may be occasions when they are not available…for instance when they are on leave. In those cases, we check to see if you are OK with someone else supporting you in the short term.

Can you provide an accessible vehicle to take me into the community?

We do have a couple of accessible vehicles available, but it’s best to get in touch with us on hello@mosaic.org.au for a chat about your transport needs.

Do you have any Support Workers that speak languages other than English?

Yes. Please get in touch on hello@mosaic.org.au and let us know which language you require, and we’ll let you know if we can help.

Does Mosaic provide any group activities in the community?

No. We don’t run group activities or provide group support options. However, we can assist you to attend other groups in your community.
