Who Do We Support?

Mosaic is one of the leading disability agencies in Perth, Western Australia (WA).

 

Our dynamic WA-based organisation delivers exceptional services is committed to enhancing the quality of life and providing social and community inclusion for Western Australians living with disability.

 

Mosaic assists people between the ages of 18 and 65. We support many types of intellectual disabilities, and additionally, many of our clients are supported for more than one disability.

 

Mosaic clients are supported in a caring and nurturing environment across all of our services by teams of highly experienced, positive and passionate support staff.

 

We are a registered provider of the National Disability Insurance Scheme (NDIS), and like all high-quality disability service agencies in WA, work to develop tailored personal life plans to ensure we always create the best possible outcomes for individuals living with a disability, enabling them to have control over their personal growth, development and quality of life.

Supported Independent Living

Our Supported Independent Living (SIL) Service includes shared accommodation for two to seven people in suburban homes within the Perth metropolitan area.

 

Shared accommodation is a great choice for people in need of cultivating their own independence with 24/7 support of friendly professionals. Mosaic homes are staffed by teams of highly experienced, positive and passionate support staff in a caring and nurturing environment.

 

Clients in our homes will be able to enjoy the benefits of a stable home life, as well as assistance in all areas of healthy living, such as: making new friends, learning new skills, setting new household routines and living in an inclusive and positive community.

 

Our around the clock service ensures that we provide clients with the level of security and assurance that is expected of high-quality disability service agencies.

Support Coordination

Some Mosaic clients have a number of different providers. We can help ease the pressure of coordinating multiple services, to get the best out of every one of them.

 

Our Support Coordination services can help clients to gain a better understanding of their NDIS plan from the moment they connect with them, and identify meaningful goals with follow-up support in reaching them.

 

We want to ensure that our clients remain happy with their services, are NDIS-ready and prepared for their reviews.

Positive Behaviour Support

The Mosaic team of qualified and dedicated specialists are skilled in developing strategies to help increase the independence of our clients. This service helps us to gain a better understanding individual behaviour.

 

The Mosaic Positive Behaviour Support service aids in developing effective strategies that may reduce and eliminate challenging behaviours or restrictive behaviours that can be putting strain on their health, support, relationships or lifestyles.
