Support Coordinators

As a Support Coordinator, you have a wonderful opportunity to achieve significant outcomes for clients. You will:

  • Work in partnership with clients (and family members and guardians) to achieve goals, aspirations and dreams;
  • Build relationships and mutual understandings with sector support coordinators, NDIS Planners and other key stakeholders;
  • Manage client funding and ensure supports are delivered in line with budgets and contractual obligations;
  • Sometimes manage a team of support workers.

Successful candidates will have:
  • Demonstrated experience in a client services coordination role in a community services environment, preferably in a supported accommodation environment;
  • Experience supporting clients with a disability, including dealing with clients with complex needs and challenging behaviours;
  • Commitment to person first and person-centred approaches to care;
  • An understanding of the NDIS;
  • A passion for working with people to ensure they get the most out of life;
  • Current Australian Driver’s Licence and right to work in Australia.

Support Workers

Successful candidates consistently assist Mosaic clients to be independent and provide person-centred support to our clients so they can achieve their dreams and aspirations. They take pride in upholding the National Disability Standards and support the Human Rights of our clients. They also want to work as part of a high performing team.

Essential Criteria
  • Minimum two years’ experience working within the Disability / Aged Care industry
  • Good verbal and written communication skills
  • First Aid Certificate (HLTAID003)
  • A National Police Clearance, no older than three months
  • An Australian Driver’s Licence and reliable vehicle
  • Full working rights in Australia

Desirable Criteria

  • Cert III or IV in Disability
  • Cert IV Mental Health
  • Cert III or IV in Community Services
  • Manual Handling Certificate from a Registered Training Organisation
